[cpp-threads] CHANGE: A hopefully clearer document on POSIX threads

A discussion has been going on that really needed to be copied to
cpp-threads.  I am sending this to all addresses, and will send
a copy of the history to cpp-threads alone in another message (so i
can be deleted in toto).
