[cpp-threads] Asynchronous Function Proposal

My apologies for not responding to this thread over the past few
days.  I have been traveling and not in a position to provide the
considered attention that the thread requires.  I am now caught up,
and most of the comments I would have made were made by someone else.
(To quote from the translation of a quote in my dissertation,
"Confound those who have said our remarks before us.")

I have tried to capture the critical portions of our discussion in
an update to the proposal.  I have probably missed some, and I have
probably spent too little text on others.  My intent is to propose
one solution, with the goal of being ready for action at the next
meeting, while still communicating issues and alternate solutions.
Comments are welcome, particularly if you feel an alternate solution
has not been adequately described, and particularly when they come
with draft text for the paper.  :-)

Anyway, I promised Herb an updated draft by Saturday, and here it is.
